Biometrics aren't foolproof, data is dirty; people change their names and sex and wear out their fingerprints doing manual labour. Only a charlatan would pretend that technology is soon bringing the day when we'll never see a case of mistaken identity. Hence

But the question is how serious the consequence is. The likeliest model seems to be the no-fly-list, and the likelihood that flaws in the identity system will create Kafkaesque mechanised discrimination with untraceable causes and no remedy. This is extremely troubling for those affected, and might have knock-on efects on trust in e-enabled services, but it's merely inconvenient for the service provider especially if (as in the case of government) it's relatively unaffected by customer difficulties and accepts no liability for inconvenience caused.
